How to transfer your medical records to another healthcare provider.

1. Log on to the the Patient Portal or Patient Information Center.

2. Click My health on the left side of the page.

3. On the My Health page, click the Forms and Documents tab.

4. Locate "Index Health: PHI Disclosure Authorization Form" or download PDF here.

5. Complete the PDF form thoroughly.

6. Sign the form

7. Send the completed document back to Index as a message attachment or you can email us at hello@indexclinic.com
